Letter 1

“Rays of golden sunlight shine through the small window in my
room. Another morning without electricity. I ponder the title of my first book,
‘Principles and practice of urban firewood cuisine.’ Inflation is at world
record levels, and meals scarce enough that my friends joke about it in their

Because transport is too expensive, I have no choice but to walk to
the hospital. At times I feel like I might evaporate in the blistering African

outpatients.

The consultant arrives early to orient us. This will probably be the
last time we receive thorough teaching. The few consultants we have now spend

a painful facial rash for the past two days. The consultant asks
what my differential diagnosis is. Before I can answer, the clearly

late cousin’s. The rest of my questions seem superfluous; the conclusion
appears obvious. “What single test would you like to do?”, the consultant asks.
Secretly I cling to the remote possibility that another cause of
immunosupression such as corticosteroid therapy, is responsible, but we all
know the answer is HIV.

Next, is a well-looking middle aged man complaining of difficulty
reading especially at night. A broad smile materialises on the senior
doctor’s face as he completes his examination. “Presbyopia - for refraction,”
he scribbles. He adds that both the diagnosis and treatment are easy and

A new patient enters the room, and the eye specialist stands up
abruptly, disturbed. He explains that conjuctival squamous cell carcinoma is
the patient’s diagnosis, and that treatment requires a complete removal of all

Guided by her son, an elderly woman, her face lined by time, greets us

sight. On examination her pupils are equal and behind them, gleaming
white pearls, shrink and enlarge as the torch lights up her eyes. “In the exam
you will not be forgiven for missing cataracts,” the ophthalmologist cautions.
She will need surgery. Due to the backlog of cases, her son is advised to check
in three months’ time. Maybe the situation with the theatres will have
improved.

He protests. The public-private dichotomy unfolds before us. Quickly
scrawled is a referral note to a private practitioner. Within one week she will
be seeing. Lucky her; she could afford.

We have seen the day’s patients. Prescribed reading in preparation for
tomorrow is ptosis, glaucoma, and the red eye. We try to read, know and
practise, but learning medicine in a pariah state is an immense challenge.
Apart from difficulties of food, water, transport, diminished teaching, and few
books, frequent strikes by doctors and nurses disrupt our tenuous education.
Morale is low as we wonder how we are ever to learn to provide appropriate
care.”

Llandudno man wins fight for life

THE family of a Llandudno man left for dead on a piece of waste ground now think he was thrown off a parapet after being savagely beaten up.

The family of Dan Lane, 24, have pieced together some of the details of the mysterious incident, which left him fighting for his life in Majorca.

Dan, from Craig-y-Don, received a fractured skull and jaw, a smashed cheekbone and eye socket, broken ribs and a punctured lung when he was beaten up in the resort of Magaluf.

And his sister Amanda Lane-Williams believes he was then dragged to a secluded part of the resort and dumped over a parapet into shrubbery.

“It’s a miracle he was found, as where he was lying was in undergrowth next to an empty hotel,” she said.

“A maintenance man who was checking the grounds before the hotel opened for the summer found him, otherwise he might not have been found in time. He had lost a lot of blood.”

When Dan’s family arrived at the hospital in Palma they were told he had little chance of survival. “They told us to expect the worst,” said Amanda.

But Dan is now making a miraculous recovery. He was flown back to North Wales by air ambulance last Wednesday, and on Sunday underwent six hours of reconstructive facial surgery at Ysbyty Glan Clwyd.

His elder brother Stephen, 32, who was on holiday with Dan at the time, said the family is traumatised, and mbt chapa he blames himself for not being with his brother when he was attacked.

They had been holidaying together in Magaluf and had spent an evening out in the resort when they became separated

Stephen and friends searched for hours but it wasn’t until 9am the next day Dan was found.
